Corporate Donors Form

Thank you for your interest in being a Corporate Donor of "United We Jam" to benefit the local artists of Houston who suffered a loss during the recent hurricane.  While many people around the world have donated blankets, clothing and food to those in need, those in our music community have also suffered the loss of music equipment, which is such a major part of their lives.  But we want to do our part and that's where we need your help.  

If you are a music company, many of those who lost music equipment are your previous customers,
so we thank you for helping them replace what has been lost. 

Your donation will be noted in all press releases associated with this event,

as well as your logo placed on all available signage. 
Plus, 100% of your donation can be tax deductible through the Matthew's Hope Foundation.
